ju ming zheng number (i tnk for singaporeans it should be the same as ur passport number. therefore, i tnk only those non-singaporeans need to give me this info)
passport expiry date
place of birth
date of birth
nationality
type of I/C
instrument (if u tnk i do not know already. HAHA)
mode of payment (cash/cheque)
amount to withdraw from edusave
next-of kin name and contact number
dietary requirements (none/vegeterian/cannibal?! blahblah, you get the idea)
